Job summary

Grace Hospice in Minneapolis, MN is seeking a dedicated Hospice Chaplain to provide individualized spiritual and emotional support to patients and families at the end of life. You will join an interdisciplinary care team and collaborate closely with nurses, physicians, social workers and other professionals to honor each patient’s beliefs and values.

The role requires a Master of Divinity (M.Div.) or equivalent, four CPE units, and an ecclesiastical endorsement.

Qualifications

• Master of Divinity (M.Div.) or equivalent theological degree from an accredited seminary or school of theology.
• Four units of Clinical Pastoral Education (CPE) required.
• Current ecclesiastical endorsement.
• Board certification preferred.
• Experience supporting individuals and families through serious illness, death, dying, grief or loss.

Responsibilities

• Provide individualized spiritual and emotional support to patients and families.
• Complete spiritual assessments and contribute to individualized plans of care.
• Offer compassionate listening, counseling, prayer and other spiritual support as desired.
• Collaborate with the interdisciplinary team to support patient and family needs.
• Connect with community clergy and faith communities when additional support is requested.
• Provide funeral or memorial support when requested by families.
• Support Grace Hospice staff through presence, conversation, prayer, devotions or memorial reflections as appropriate.
• Complete required documentation and maintain communication with the Chaplain team and supervisor.
