Job summary

Grace Hospice MN in Minneapolis, MN is seeking a compassionate Hospice Chaplain to join our interdisciplinary care team. This full-time role provides individualized spiritual and emotional support to patients and families, guiding care plans and offering comfort through listening, prayer, and counseling.

Collaborate with nurses, physicians, social workers and other professionals to meet patient needs. Requires M.Div., 4 CPE units, and ecclesiastical endorsement; board certification preferred.

Qualifications

• Master of Divinity (M.Div.) or equivalent theological degree.
• Four units of Clinical Pastoral Education (CPE) required.
• Current ecclesiastical endorsement.
• Board certification preferred.
• Experience supporting individuals and families through serious illness, death, grief or loss.

Responsibilities

• Provide individualized spiritual and emotional support to patients and families.
• Complete spiritual assessments and contribute to individualized plans of care.
• Offer compassionate listening, counseling, prayer and other spiritual support as desired.
• Collaborate with the interdisciplinary team to support patient and family needs.
• Connect with community clergy and faith communities when additional support is requested.
• Provide funeral or memorial support when requested by families.
• Support Grace Hospice staff through presence, conversation, prayer, devotions or memorial reflections as appropriate.
• Complete required documentation and maintain communication with the Chaplain team and supervisor.

Grace Hospice is looking for a compassionate Hospice Chaplain to join our interdisciplinary care team.

As a Hospice Chaplain, you’ll provide individualized spiritual and emotional support to patients and families based on their beliefs, values and wishes. You’ll build meaningful connections, offer a comforting presence, and help patients and loved ones navigate the spiritual and emotional aspects of the hospice journey.

You’ll also collaborate with nurses, physicians, social workers, hospice aides and other professionals who share a commitment to providing care with kindness, dignity and respect.

What We’re Looking For

• Master of Divinity (M.Div.) or equivalent theological degree from an accredited seminary or school of theology
• Four units of Clinical Pastoral Education (CPE) required
• Current ecclesiastical endorsement
• Board certification preferred
• Experience supporting individuals and families through serious illness, death, dying, grief or loss
• Ability to provide a compassionate, nonjudgmental listening presence
• Respect for different cultures, traditions, beliefs and values
• Strong communication and relationship-building skills
• Ability to work independently while collaborating as part of an interdisciplinary team
• Ability to creatively connect with individuals experiencing Alzheimer’s disease or other forms of cognitive decline
