Job summary

1057 AdventHealth Home Health and Hospice Inc is seeking a Chaplain to provide spiritual care for hospice patients and families. You will assess needs, design care plans, and collaborate with physicians, social workers, and clergy to support the patient’s spiritual well-being.

Responsibilities include Bereavement support, coordinating with chaplaincy networks, and maintaining thorough documentation in the patient database. Requires Master’s in Divinity and SDA employment eligibility.

Qualifications

• Knowledge of counseling theories and practices compliant with ethical and legal parameters.
• Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and internet tools.
• Experience with patient database systems (Homeworks).
• Ability to respect cultural and religious differences.

Responsibilities

• Provide spiritual support and counseling to hospice patients, caregivers, and families.
• Clarify spiritual welfare concerns and arrange additional support as needed.
• Act as liaison with religious leaders and communities related to patient care.
• Perform initial pastoral care assessments within 5 days of admission.
• Design and oversee pastoral care plans implementation.
• Make bereavement calls within 2 days of patient death.
• Provide bereavement support and coordinate with bereavement staff.
• Participate in interdisciplinary team meetings and maintain documentation.
