Job summary

Optum, part of UnitedHealth Group, is hiring a Registered Nurse Case Manager for our hospice program in Bowling Green, Missouri. You will oversee the Plan of Care, collaborate with the IDG, and ensure compassionate, high-quality end-of-life care for patients and families.

Qualifications

• Current and unrestricted MO RN license.
• Valid driver's license and access to reliable transportation.
• Ability to provide compassionate care across diverse home environments.

Responsibilities

• Performs initial nursing evaluation for hospice eligibility within 48 hours of referral.
• Develops and coordinates the Plan of Care with the patient/family and IDG.
• Documents goals, interventions, and patient responses to care.
• Educates and supports patient/family in self-care and disease management.
• Maintains accurate clinical notes and adapts care to patient needs.
