Job summary

Elevation Hospice of Colorado is seeking a dedicated Registered Nurse to provide skilled nursing care to patients in their homes. The role involves coordinating with the interdisciplinary team, educating families, and ensuring high-quality end-of-life care in a compassionate setting.

The ideal candidate will have an active Colorado RN license, CPR certification, and at least two years of RN experience, preferably in hospice or home health, with strong communication and organizational skills.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current RN license in Colorado.
• Two years’ RN experience; hospice/home health preferred.
• Current CPR certification.
• Proficient computer skills; MS Office experience preferred.
• Strong interpersonal, communication, and organizational skills.
• High ethical standards and patient-family care focus.

Responsibilities

• Perform initial and ongoing assessments of patient condition, needs, and ADLs.
• Coordinate care with the interdisciplinary team and family.
• Educate patients and families on disease process and end-of-life care.
• Initiate and document care plans and revisions as needed.
• Provide on-call nursing coverage for assigned patients.
• Coordinate services and referrals with IDT members as needed.
• Notify physician of changes in patient status and responses to care.
• Assess caregiver ability to meet patient needs and adjust plans accordingly.
• Communicate information using available organizational systems.
