Job summary

VitalCaring is seeking an Experienced Clinical Care Manager (RN) – Hospice to lead and coordinate comprehensive end-of-life care for patients and families in the Round Rock area.

You will guide the care team, oversee plans of care, and ensure compassionate, evidence-based practice. The role emphasizes communication, collaboration, and adherence to hospice regulations while maintaining quality outcomes.

Qualifications

• Active RN license in state of practice.
• BSN preferred from an accredited program.
• Hospice or Home Health experience required.
• 2+ years in leadership, supervisory, or care coordination roles preferred.
• Ability to collaborate in an interdisciplinary care team.
• Strong clinical judgment for symptom management and patient comfort.
• Excellent communication with patients, families and care teams.
• Valid driver’s license and reliable transportation, CPR current.

Responsibilities

• Coordinate hospice patient care across the interdisciplinary team and contracted providers.
• Serve as central communication point between physicians, caregivers, patients and internal teams.
• Manage physician orders for timely entry, approval and alignment with the care plan.
• Partner with Intake to support admissions and follow up on referrals.
• Continuously assess patient needs and trigger reassessments with any change.
• Lead development and updates of individualized hospice plans of care.
• Support patients and families through education, symptom control and end-of-life planning.
• Review clinical reports and take appropriate action; participate in IDG meetings.
