Care Initiatives Hospice seeks an RN Clinical Manager to lead end‑of‑life care teams with compassion and expertise. You’ll supervise clinical staff, coordinate care across home, long‑term care, and hospital settings, and ensure personalized plans that honor patient wishes.

The role emphasizes hospice/palliative care experience, leadership skills, and the ability to work across settings to support patients and families during this significant stage of life.

Qualifications

• Active RN license or ability to activate in the state(s) you operate.
• Active CPR certification.
• Hospice or palliative care experience.
• At least one year in a leadership or charge nurse position or equivalent.
• Two years of recent clinical experience in hospice, oncology, home health or public health nursing; hospice experience preferred.

Responsibilities

• Lead clinical teams to deliver comprehensive, compassionate end‑of‑life care.
• Meet patients and families where they are, in homes, long‑term care communities, or facilities.
• Collaborate with caregivers to honor patient needs, wishes, and goals of care.
