Job summary

Journey Hospice is seeking a per diem as needed CHHA for our Journey Hospice Program. This position serves terminally ill patients in the home, nursing facilities, inpatient units, or other environments where the Hospice patient resides, in accordance with the plan of care and under RN supervision.

The Hospice CHHA provides personal care and light housekeeping, observes and reports observations, and maintains timely documentation.

Qualifications

• High school diploma or GED required.
• Certification as Hospice Aide/Homemaker per NJ requirements.
• Valid driver's license with reliable transportation.

Responsibilities

• Provide personal care and light housekeeping to terminally ill patients.
• Observe, report, and document patient status and care provided.
• Participate as an active member of the Hospice Interdisciplinary Team (IDT).
