Vitality Hospice invites a dedicated Certified Home Health Aide (CHHA) to join our clinician-owned team. This field-based, full-time role supports patients in their homes, promoting comfort, dignity, and quality of life at end-of-life.

Under RN supervision, you’ll assist with mobility, personal care, companionship, and light housekeeping. Travel is required with mileage reimbursed; active CHHA certification and a valid driver's license are essential.

Qualifications

• Active CHHA certification is required.
• Valid driver's license, reliable vehicle, and current auto insurance.
• Compassionate, reliable, and self-directed.
• Strong communication and time-management skills.
• Ability to work independently and follow care plans.

Responsibilities

• Assist with mobility, transfers and ambulation.
• Provide personal care (bathing, grooming, dressing, feeding).
• Offer companionship and emotional support.
• Perform light housekeeping tasks.
• Document care accurately and in a timely manner.
