Job summary

LifeSource Hospice is seeking a dedicated Hospice Certified Home Health Aide (CHHA) to provide compassionate personal care to patients in private homes, assisted living facilities, and long-term care settings. This field-based role requires regular travel between patient locations under RN supervision.

Responsibilities include bathing, mobility assistance, documenting visits, and maintaining patient dignity in end-of-life care, while upholding safety and infection control standards.

Qualifications

• Current CHHA certification in New Jersey is required.
• Previous hospice, home health, long-term care, or assisted living experience preferred.
• Ability to work independently in the field with strong time management.

Responsibilities

• Provide personal care including bathing, grooming, toileting, and mobility assistance.
• Assist with transfers and mobility following safety protocols.
• Observe and report changes to supervising nurse promptly.
• Provide emotional support and companionship during end-of-life care.
• Maintain accurate documentation of patient visits and care provided.
• Follow infection prevention and control procedures at all times.
• Assist with light homemaking tasks related to patient care as directed.
• Communicate professionally with nurses, facility staff, families, and team members.
• Travel daily between patient homes, facilities, and care settings within coverage area.
• Maintain compliance with hospice regulations and state CHHA requirements.
