Crossbridge Hospice in Akron, OH is seeking a compassionate Hospice Aide to provide personal care and supportive services in patients’ homes. The role emphasizes dignity, comfort, and quality of life within an individualized Plan of Care.

Ideal candidates are CNAs or STNAs, with a valid driver's license and reliable vehicle, able to collaborate within a multidisciplinary team and work in field settings to support patients and families during end-of-life care.

Qualifications

• Must be a certified nurse’s aide (CNA). State Tested Nursing Assistant (STNA) for Ohio.
• Must maintain a valid driver’s license, good driving record and reliable automobile.
• High school graduates or GED preferred.
• Demonstrate the ability to be able to work within a multi-disciplinary team setting.
• Demonstrate the ability to work in field situations.

Responsibilities

• The Hospice Aide provides compassionate personal care and supportive services to patients in their place of residence.
• Assists individuals who are nearing end of life, helping maintain dignity, comfort, and quality of life.
• The Hospice Aide is an essential member of the care team and delivers care in alignment with the patient’s individualized Plan of Care.
• Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care.
