Qualifications

• Must be a CNA with active certification.
• Must maintain a valid driver’s license and reliable vehicle.

• Ability to perform field/home visits.

Responsibilities

• Provide compassionate personal care and supportive services to patients in their home.
• Assist patients with at least two ADLs, including personal hygiene.
• Assist with medication intake and prescribed exercises as directed.
• Perform incidental household services essential to patient care.
• Follow the patient’s Plan of Care and update care plans as needed.
• Apply 4Ms framework and deliver Age-Friendly Care.

How You Will Contribute

The Hospice Aide provides compassionate personal care and supportive services to patients in their place of residence. This role assists individuals who are nearing end of life, helping maintain dignity, comfort, and quality of life. The Hospice Aide is an essential member of the care team and delivers care in alignment with the patient’s individualized Plan of Care.

How You Will Make An Impact

• Provide assistance with at least 2 of the following ADL's including personal hygiene
• Assist patients with medication intake and prescribed exercises
• Incidental household services, which are essential to patient health care
• Will follow the Plan of Care (Instruction Sheet) as instructed and make appropriate updates during scheduled visits
• Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care

The Experience And Skills You Bring

• Must be a certified nurse’s aide (CNA). State Tested Nursing Assistant (STNA) for Ohio
• Must maintain a valid driver's license, good driving record and reliable automobile
• High school graduates or GED preferred
• Demonstrate the ability to be able to work within a multi-disciplinary team setting
• Demonstrate the ability to work in field situations

Preferred Knowledge, Skills, And Experience

• Preferred candidates will have in-home and/or hospice experience
