The ideal candidate will have a Florida CNA license, CPR/BLS certification, and reliable transportation, with flexibility to work weekends and holidays as needed.

Aufgaben

• Provide personal care services in accordance with the plan of care and hospice regulations.
• Assist with patient ambulation and exercise routines.
• Perform basic procedures as directed by licensed clinicians.
• Observe and report changes in the patient’s condition to a licensed nurse.
• Complete timely and accurate documentation per policy.
• Educate family or caregiver on proper patient care techniques.
• Maintain confidentiality of all patient information.
• Participate in interdisciplinary team meetings and other required sessions.
• Complete a minimum of 12 hours of job-specific training annually.
• Perform other duties as assigned.
