Job summary

Community Hospice is seeking a compassionate Hospice Aide (CNA) to provide hands-on personal care to terminally ill patients across multiple sites. The role emphasizes patient comfort, dignity, and safety in accordance with the plan of care.

The position requires assisting with mobility, supporting families, and flexible weekend/holiday coverage. Strong documentation, communication, and teamwork are essential to ensure high-quality hospice care.

Qualifications

• Understanding of personal care principles and hospice care standards.
• Ability to communicate effectively with patients, families, and team members.
• Demonstrated ability to work as part of an interdisciplinary team.
• Strong organizational and documentation skills.
• Minimum six months of relevant experience preferred.

Responsibilities

• Provide personal care services in accordance with the plan of care and hospice regulations.
• Assist with patient ambulation and exercise routines.
• Perform basic procedures as directed by licensed clinicians.
• Observe and report changes in the patient’s condition to a licensed nurse.
• Complete timely and accurate documentation per policy.
• Educate family or caregiver on proper patient care techniques.
• Maintain confidentiality of all patient information.
• Participate in interdisciplinary team meetings and other required sessions.
• Complete a minimum of 12 hours of job-specific training annually.
• Perform other duties as assigned.
