Job summary

Community Hospice seeks a dedicated Hospice Aide to provide compassionate, hands-on personal care to terminally ill patients across multiple care sites. You will support patient comfort, dignity, and safety while adhering to the plan of care, assist with mobility, and help family caregivers.

Weekend and holiday coverage and cross-site flexibility are required. Applicants should have a Florida CNA license, CPR/BLS certification, and a high school diploma or GED.

Qualifications

• Understanding of personal care principles and hospice care standards.
• Ability to communicate effectively with patients, families, and team members.
• Demonstrated ability to work as part of an interdisciplinary team.
• Strong organizational and documentation skills.
• Experience in hospice or terminal care preferred.

Responsibilities

• Provide personal care services in accordance with the plan of care and hospice regulations.
• Assist with patient ambulation and exercise routines.
• Observe and report changes in the patient’s condition to a licensed nurse.
• Complete timely and accurate documentation per policy.
• Educate family or caregiver on proper patient care techniques.
• Maintain confidentiality of all patient information.
• Participate in interdisciplinary team meetings and required sessions.
• Complete a minimum of 12 hours of job-specific training annually.
