Qualifications

• Current CNA/HHA certification and LTC certification.
• CPR certification required.
• Driver’s license and auto insurance required.
• HomeCare HomeBase experience preferred.

Responsibilities

• Provide direct patient care under the RN and plan of care.
• Assist with safe transfers and ambulation per policy.
• Document care rendered and report changes to supervisor.
• Maintain infection control and a clean, safe environment.
• Communicate respectfully with patients, families, and staff.
