Job summary

Heart to Heart Hospice in Texas is seeking a compassionate Hospice Aide (CNA) to provide direct patient care under the supervision of an RN. You will assist with transfers, ambulation, and reporting changes to ensure continuity of care, while following infection control principles and agency policies.

The role requires a current CNA license, high school diploma or GED, and at least one year of hospice or nursing assistant experience.

Qualifications

• High School Diploma preferred or GED.
• Unencumbered CNA license required.
• Current CNA licensure in applicable registry.
• At least one year as Hospice Aide/Nursing Assistant preferred.
• Owns valid driver's license and reliable transportation.
• EMR experience is a plus.

Responsibilities

• Provide direct patient care under RN direction per Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report and document changes to ensure continuity of care.
• Practice infection control principles.
• Communicate with patients, families, and teammates respectfully.
• Submit required documentation in a timely manner.
• Perform other duties as assigned and ensure regulatory compliance.
