Job summary

Heartland Hospice seeks a compassionate Hospice Aide/CNA to provide hands-on personal care and emotional support to patients at home, in assisted living, or inpatient units.

Requirements include an active CNA license, current CPR, and reliable transportation. Six months of direct patient care experience and comfort with home settings are expected, with independent work and teamwork as part of daily care.

Qualifications

• Active CNA license in the employed state is required.
• Current CPR certification is required.
• Driver's license and reliable vehicle with insurance are required.
• Six months of direct patient care experience required.
• Experience in home settings, hospice facilities, or long-term care environments preferred.
• Demonstrated compassion, patience, and ability to handle emotional situations.
• Willingness to work independently and adapt to changing patient needs.

Responsibilities

• Provide direct personal care to patients at home.
• Assist with hygiene, grooming, and toileting.
• Support safe mobility with walkers and wheelchairs.
• Assist with meals and light housekeeping.
• Observe and report health changes.
• Provide emotional support to patients and families.
• Document visits accurately and timely.
• Attend team meetings and contribute to quality improvement.
• Follow infection control and safety protocols.
