Job summary

Elara Caring is seeking a Hospice Aide to join our team covering Monroe County, GA. You will provide skilled nursing support per the Plan of Care, monitor patient condition, and collaborate with the RN Case Manager to adjust care as needed.

You'll educate patients and caregivers, document medications, and help prevent hospitalizations through coordinated in-home care, while maintaining compassionate communication with families and the care team.

Qualifications

• Understanding of hospice care principles and patient comfort.
• Strong communication with patients, families, and care teams.
• Ability to work collaboratively with RNs and other staff.
• Attention to accurate documentation and medication profiles.
• Physical ability to lift up to 50 pounds and assist transfers.

Responsibilities

• Provide skilled nursing care per the Plan of Care, physician orders, and RN direction.
• Monitor patient condition and report changes to the RN Case Manager, physician, and care team.
• Collaborate with the RN Case Manager to update the Plan of Care as needed.
• Educate patients and caregivers on self-care to support safety, independence, and overall health.
• Document medication regimens and maintain up-to-date medication profiles.
• Help prevent avoidable hospitalizations through proactive, coordinated patient care.
• Provide compassionate, high-quality care while maintaining positive communication with patients, families, physicians, and the care team.

As a Hospice Aide., you’ll contribute to our success in the following ways:

• Provide skilled nursing care according to the individualized Plan of Care, physician orders, and direction of the RN.
• Monitor patient condition and promptly report changes, concerns, or safety issues to the RN Case Manager, physician, and care team.
• Collaborate with the RN Case Manager to address patient needs and recommend updates to the Plan of Care when appropriate.
• Educate patients and caregivers on self-care techniques to support safety, independence, and overall health.
• Document medication regimens and maintain accurate, up-to-date medication profiles.
• Help prevent avoidable hospitalizations and emergency room visits through proactive, coordinated patient care.
• Provide compassionate, high-quality care while maintaining positive communication with patients, families, physicians, and the care team.

What is Required?

• High School Diploma or GED equivalent required
• 1 year of experience as a Hospice Aide or Nursing Assistant is required
• State-established or other Hospice Aide training program that meets the requirements of 42 CFR §484.36 and a competency evaluation program
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a home health environment is preferred
