Job summary

Suncoast Hospice of Hillsborough, part of Empath Health, seeks a Care Team Assistant to support interdisciplinary teams, patients and families in Brandon, FL. The role emphasizes administrative duties, chart accuracy, and coordinating with care teams and community partners.

You will handle clerical tasks, communications, and event support while upholding Empath Health's standards of care and compassion.

Qualifications

• High school diploma or GED required.
• 2 years administrative hospice or related field preferred.
• Excellent verbal and written communication skills.
• Solid customer service skills.
• Excellent documentation and organizational abilities including time management and multi-tasking.
• Ability to work independently and with diverse teams, including volunteers.
• Proficient in MS Word, Excel and Outlook; willingness to learn more software.
• Medical terminology background preferred.

Responsibilities

• Perform administrative clerical duties (filing, entering, copying documents).
• Distribute mail, faxes and phone messages promptly.
• Maintain office and team supplies.
• Ensure accuracy of patient charts.
• Distribute paperwork to disciplines and community partners.
• Assist leadership for weekly IDT meetings and related events.
• Track team census and updates in electronic systems.
• Coordinate with staff to cover patient care needs and handle calls professionally.
