Job summary

Suncoast Hospice, a part of Empath Health, seeks a Care Navigator (Customer Service Specialist) to coordinate service inquiries and provide prompt access within the Empath Health Network of Care. You’ll triage inquiries by phone, website, fax and walk-ins, guiding toward the best program for each patient’s needs.

You will input referrals into the EMR in real time, protect patient confidentiality, and support families with calm, empathetic service. A bilingual ability is preferred.

Qualifications

• High school diploma or GED; or related experience.
• Excellent verbal and written communication with patients, families and staff.
• Bilingual preferred.
• Strong multi-tasking and critical thinking skills.
• HIPAA knowledge and healthcare terminology.

Responsibilities

• Provide access to Empath Health Care Continuum via phone, website, fax and walk-ins.
• Triage inquiries using Care Navigation protocols to the best program.
• Input referrals into EMR accurately and in real time.
• Coordinate with patients, families and providers to gather information for referrals.
• Maintain confidentiality and provide empathetic, compassionate service.

Bilingual preferred.Excellent multi-tasking and critical thinking skills.Knowledge of Microsoft Office products (MS Outlook, Word, Excel). Home Care Home Base preferred.
