Job summary

Providence at Home with Compassus seeks a licensed Speech-Language Pathologist to plan and provide treatment for patients with communication disorders in a home health setting. You will perform histories and exams, document within 24 hours of visits, and develop individualized care plans aligned with patient goals.

Responsibilities include coordinating care, ensuring Medicare/Medicaid compliance, and delivering safe, effective interventions.

Qualifications

• Master's degree in Speech-Language Pathology or equivalent.
• California license required or to be obtained within 30 days of hire.

Responsibilities

• Assess SLP care needs after referral and perform accurate history and exam.
• Document assessment data within 24 hours of home visit.
• Describe assessment data and interpret findings holistically.
• Determine eligibility for reimbursement and assist in obtaining information for alternative payment sources.
• Coordinate care and develop a written plan of care meeting client goals.
• Provide SLP interventions safely and according to the plan of care.
