Job summary

Providence at Home with Compassus in Eureka, CA is seeking a licensed Speech-Language Pathologist to plan and provide home-based care for patients with communication disorders. You will assess needs, develop and implement patient-centered care plans, and document progress in accordance with Medicare and SHC guidelines.

You will coordinate with families and the care team, deliver interventions safely, and ensure timely visits, with regular reviews every 60 to 90 days as patient conditions change.

Qualifications

• Must hold a current California Speech-Language Pathologist license.
• Master's in Speech-Language Pathology required; home health experience preferred.

Responsibilities

• Assess SLP needs after referral and perform appropriate history/physical exam.
• Document assessment data within 24 hours of home visits.
• Develop and revise care plans with measurable goals.
• Coordinate care with patients, families, and insurance providers.
• Deliver treatment interventions safely and in accordance with plan of care.
