Job summary

Compassus is seeking a Home Health Speech Language Pathologist to deliver evidence-based communication and swallowing therapy in patients' homes, aligned with the Compassus Pillars of Success. The role emphasizes collaboration with physicians, families, and the care team to promote quality outcomes and patient satisfaction.

The SLP will develop plans of care, perform assessments, educate stakeholders, and document care in the EMR, while participating in ongoing team meetings and program

Qualifications

• Bachelor's degree in Speech Language Pathology required.
• Master's degree in Speech Language Pathology preferred.
• Minimum of one year of experience in a home health setting required.

Responsibilities

• Develops speech therapy plans of care after physician review.
• Conducts patient visits for assessment and evaluation; communicates with physician as needed and completes OASIS and related documentation.
• Documents care and maintains accurate medical records; submits paperwork timely.
• Educates patients, families, and care team members on therapies and progression.
• Participates in interdisciplinary team meetings and quality improvement activities.
