Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate, direct and provide speech-language pathology services to patients in the home or facility, reporting to the Clinical Manager. You will participate in the development and periodic review of the Plan of Treatment and Plan of Care.

Using professional skills to assess and treat disorders of speech, voice, language, hearing and swallowing, you will also observe, record and report changes and provide counselling

Qualifications

• Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA).
• Minimum six months experience as a Speech-Language Pathologist.
• Current and unrestricted professional license.
• Current CPR certification.

Responsibilities

• Evaluate, direct and provide speech-language pathology services in the home or facility.
• Participate in development and review of Plans of Treatment and Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer diagnostic tests and interpret results for therapy.
• Document changes and progress; provide reports to supervisor/physician.
• Instruct patients and families on communication supports and training.
• Coordinate care and participate in discharge planning.
