Job summary

Partners In Care in Oregon seeks a Support Registered Nurse to plan, coordinate, implement, and evaluate Home Health services, with cross-training in Hospice. You will provide cost-effective, high-quality patient care in patients' homes, discuss plans with families, and work with the care team to optimize outcomes.

The role requires an Oregon RN license, strong communication, and the ability to manage care independently while collaborating with interdisciplinary teams.

Qualifications

• Oregon RN license required.
• Background check including MVR, drug screen, and TB test.
• Graduate of an accredited nursing program.
• 1–2 years of home health or hospice preferred.
• Current automobile insurance and valid driver’s license.
• Valid CPR certification.

Responsibilities

• Perform initial and ongoing home health assessments and document findings.
• Develop and update individualized care plans with patient and family involvement.
• Administer treatments and medications as prescribed.
• Coordinate equipment and services needed for care.
• Educate patients and families with appropriate teaching methods.
• Coordinate with the case manager and physician; provide updates as needed.
• Maintain and update the plan of care, including supervisory visits every 14 days.
• Communicate with all care team members to ensure coordinated care.
• Complete assessments promptly after admission and continuous documentation.
• Participate in on-call duties and after-hours visits per policy.
• Local/regional travel within Central Oregon by personal vehicle.
