Job summary

Partners In Care is seeking a Support Registered Nurse to plan, coordinate, implement, and evaluate hospice services with potential cross‑training in Home Health. You will provide nursing care in patients' homes and facilities across Central Oregon, ensuring quality outcomes.

We value empathy, collaboration, and patient‑centered care. The role is full‑time, 8:00 AM–5:00 PM, with travel throughout Central Oregon and occasional after‑hours on-call requirements.

Qualifications

• Unencumbered Oregon RN license required.
• Ability to pass a background test, including MVR, drug screen, and TB test.
• Graduate of an accredited nursing school.
• One to two years of recent medical care experience; home health or hospice preferred.
• Current automobile insurance and a valid Oregon driver’s license.
• CPR certification.
• Strong communication and interpersonal skills.

Responsibilities

• Plan, coordinate, implement, and evaluate hospice services; cross-train in home health.
• Provide comprehensive physical assessments and health history; determine care needs.
• Develop individualized care plans with goals based on nursing diagnoses.
• Administer medications and treatments per physician orders.
• Coordinate patient care with case manager and other healthcare team members.
• Participate in on-call duties and after-hours visits as required.
• Document and communicate patient status updates and care plans to families and facilities.
