Job summary

Affinity Hospice Care Inc. is seeking a per-diem Registered Nurse to support home-based hospice services in Oregon.

You will review OASIS assessments, ensure accurate ICD-10 coding, and collaborate with clinical staff to maintain data integrity while prioritizing patient safety and comfort in their home settings. The role requires an active California RN license, 1–2 years of home health experience, and CPR certification, with strong communication and independent work skills.

Qualifications

• 1-2 years of clinical home health experience.
• Active Californian RN license in good standing.
• Working knowledge of OASIS assessments and ICD-10 coding.
• Knowledge of federal regulations and state licensure requirements.
• Strong communication skills and attention to detail.

Responsibilities

• Review OASIS assessments for completeness and compliance.
• Ensure accurate ICD-10 coding and sequencing.
• Partner with clinical staff to resolve documentation issues.
• Monitor visit utilization and flag financial risks.
• Identify trends impacting agency outcomes.
• Conduct OASIS inter-rater reliability visits.
• Participate in Quality Improvement activities.
• Stay current with best practices through ongoing education.
• Perform other duties as required for patient services.
