Job summary

ChristianaCare HomeHealth is seeking a compassionate Registered Nurse (RN) to care for primarily geriatric patients in their homes. You will perform skilled nursing, assessments, and care planning while supporting independence and comfort in a home setting.

The role offers part-time day shifts with one weekend per month, two holidays per year, and limited on-call responsibilities. A valid Delaware RN license, 1+ year of RN experience, and a driver’s license are required.

Qualifications

• Current RN licensure in Delaware or eligibility through a Compact State License.
• BSN preferred; ADN candidates may be considered with a commitment to obtaining a BSN within the designated timeframe.
• Minimum of one year of RN experience.
• Home Health, Medical-Surgical, Geriatric, or Community Health experience preferred.
• IV therapy and wound care experience preferred or willingness to complete training.
• Current valid driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing care and prescribed treatments to predominantly geriatric patients in their homes in and around North Wilmington, DE.
• Perform comprehensive patient assessments and ongoing evaluations.
• Develop, implement, and update individualized care plans.
• Administer medications, treatments, and IV therapy as ordered.
• Educate patients and caregivers regarding disease management, medications, safety, and wellness.
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
• Facilitate discharge planning and transitions of care.
