ChristianaCare HomeHealth is seeking a compassionate Registered Nurse (RN) to join our dedicated team caring for primarily geriatric patients throughout the community. You will have autonomy to manage your daily schedule while being supported by an interdisciplinary team in patients' homes around North Wilmington, DE.

The role emphasizes skilled nursing, IV therapy, patient education, and care coordination with physicians and other professionals, with a part-time, day-shift schedule and limited

Qualifications

• Current RN licensure in Delaware or eligibility through a Compact State License.
• Home Health, Medical-Surgical, Geriatric, or Community Health experience preferred.
• BSN preferred; ADN candidates may be considered with commitment to obtaining BSN within designated timeframe.

Responsibilities

• Provide skilled nursing care and prescribed treatments to predominantly geriatric patients in their homes in and around North Wilmington, DE.
• Perform comprehensive patient assessments and ongoing evaluations.
• Develop, implement, and update individualized care plans.
• Administer medications, treatments, and IV therapy as ordered.
• Educate patients and caregivers regarding disease management, medications, safety, and wellness.
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
• Facilitate discharge planning and transitions of care.
