Job summary

ChristianaCare HomeHealth is seeking a compassionate Registered Nurse to join our team caring for geriatric patients in their homes around North Wilmington, DE. You’ll work autonomously within an interdisciplinary team, delivering skilled care and personalized support to help patients maintain independence.

The role emphasizes comprehensive assessments, evidence-based care planning, IV therapy, education, and coordination with physicians and therapists in a community setting.

Qualifications

• Current RN licensure in Delaware or eligibility through a Compact State License.
• BSN preferred; ADN candidates may be considered with commitment to BSN.
• Minimum of one year of RN experience.
• Home Health, Medical-Surgical, Geriatric, or Community Health experience preferred.
• IV therapy and wound care experience preferred or willingness to complete training.
• Current valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled nursing care to predominantly geriatric patients in their homes in and around North Wilmington, DE.
• Perform comprehensive patient assessments and ongoing evaluations.
• Develop, implement, and update individualized care plans.
• Administer medications, treatments, and IV therapy as ordered.
• Educate patients and caregivers regarding disease management, medications, safety, and wellness.
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
• Facilitate discharge planning and transitions of care.
