Job summary

ChristianaCare HomeHealth in Delaware seeks a compassionate Home Health Registered Nurse to deliver skilled nursing care in patients' homes, build trusting relationships, and promote independence across Wilmington, Newark and surrounding communities. This role emphasizes one-on-one care and a patient-centered approach.

You will work day shifts, three weekends per month (12 hours Sat-Sun), two holidays per year, and two on-call shifts monthly; extensive benefits, tuition assistance, retirement

Qualifications

• BSN required.
• ADN candidates must obtain BSN within three (3) years of their hire date.
• Current Delaware RN license or Compact State RN license.
• Minimum of one (1) year of RN experience.
• IV proficiency or willingness to complete training and perform IV-related functions.

Responsibilities

• Deliver skilled nursing care and prescribed treatments to geriatric patients in their homes and throughout the community.
• Build trusting relationships with patients and caregivers while promoting health, independence, and quality of life.
• Develop and implement individualized, patient-centered care plans that are goal-oriented and aligned with available resources.
• Coordinate care with physicians, caregivers, and interdisciplinary team members to achieve the best possible outcomes.
• Develop and execute discharge plans that ensure continuity of care.
• Perform IV therapy and related nursing functions. New team members will receive training and are expected to achieve IV proficiency within 90 days of hire.
