Job summary

ChristianaCare HomeHealth is seeking a compassionate Registered Nurse (RN) to provide skilled nursing care to predominantly geriatric patients in their homes in and around North Wilmington, DE. You will perform assessments, develop and update care plans, administer medications and IV therapy, and educate patients and caregivers.

Qualifications

• Current RN licensure in Delaware or eligibility through a Compact State License.
• BSN preferred; ADN candidates may be considered with a commitment to obtaining a BSN within the designated timeframe.
• Minimum of one year of RN experience.
• Home Health, Medical-Surgical, Geriatric, or Community Health experience preferred.
• IV therapy and wound care experience preferred or willingness to complete training.
• Current valid driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing care and prescribed treatments to predominantly geriatric patients in their homes in and around North Wilmington, DE.
• Perform comprehensive patient assessments and ongoing evaluations.
• Develop, implement, and update individualized care plans.
• Administer medications, treatments, and IV therapy as ordered.
• Educate patients and caregivers regarding disease management, medications, safety, and wellness.
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
• Facilitate discharge planning and transitions of care.
