• EMR experience preferred.

Responsibilities

• Care Management and Service Delivery - provides ongoing assessment and nursing care to patients in homes and health care facilities; develops and implements a care plan.
• Quality and Compliance Management - audits and maintains patient records; participates in quality improvement projects.
• Oversees plan of care implementation and evaluates effectiveness; communicates with other health care providers.
• All other duties as assigned.
