Job summary

Independence Home Health LLC in Edinburgh, IN, is seeking a Registered Nurse Case Manager to join our home health team. You will assess patients, develop care plans, coordinate with physicians and families, and supervise support staff.

Qualifications

• Graduate of an accredited nursing program.
• Active Indiana RN license and CPR certification.
• Minimum of 2 years nursing experience; home health experience preferred.
• Strong clinical, communication, leadership and organizational skills.

Responsibilities

• Conduct patient assessments and develop individualized plans of care.
• Coordinate care with physicians, patients, families, and other healthcare professionals.
• Educate patients and caregivers on care plans and treatments.
• Supervise and support LPNs, HHAs, and CNAs for quality care.
• Review and update plans of care as needed.
• Maintain accurate, timely clinical documentation.
• Participate in case conferences and quality improvement initiatives.
• Ensure safe, compassionate in-home care.
