Compass Park in Johnson County near Indianapolis seeks an RN Case Manager to support our Home Health team covering Johnson and Bartholomew counties. The role includes collaborating with the Director of Nursing and coordinating a comprehensive care plan for home-based clients.

The position requires an Indiana-licensed RN with at least two years of nursing experience (home care preferred) and the ability to supervise RNs, LPNs, HHAs, and PCAs while maintaining paperless documentation and

Qualifications

• Graduate of an accredited school of professional nursing.
• Current Indiana RN license.
• Minimum of two years nursing experience; home care preferred.
• Ability to supervise and direct members of the client care team.
• Paperless documentation capability.
• Valid driver's license and insured automobile.

Responsibilities

• Reviews client referrals and responds to requests and inquiries.
• Collaborates with the Director of Nursing on eligibility for home care.
• Completes comprehensive assessments including OASIS within 48 hours.
• Performs initial and ongoing client assessments per policy and practice.
• Collaborates with physicians, therapists, social services, and families to plan care.
• Manages/supervises RNs, LPNs, HHAs, and PCAs to deliver quality home care.
• Promotes safety and confidentiality; ensures compliance with CHAP and state/federal rules.
• Performs other related duties and completes tasks within guidelines.

This job covers Johnson and Bartholomew counties.Full Time and Part Time availableRN Case Manager:Reviews client referral information and responds to requests and inquires as appropriate Collaborates with the Director of Nursing regarding the eligibility and appropriateness of client for home care servicesCompletes comprehensive assessments including OASIS data set within 48 hours of referralPerforms initial and ongoing client assessments based on agency policy and standards of practice to ensure effective and appropriate home care services. Also considers the physical, psychological, and socioeconomic needs of the client, the level of family acceptance, and the ability for client needs to be met in the homeCollaborates with physicians, other health care professionals (therapists, social services, pastoral care, supportive services), clients, and families in developing comprehensive, coordinated plan for careManages/supervises a team of RNs, LPNs, HHAs, and PCAs to provide effective and quality home care services Promotes personal safety and a safe environment for clients and coworkers and provides care in accordance with state, federal, and CHAP regulationsMaintains confidentiality in all aspects of the job.
