Job summary

This full-time/part-time position offers independence in caseload management and opportunities to build lasting relationships with patients and families within a trusted local agency.

Qualifications

• Active Indiana RN license.
• ASN or BSN required.
• Home health experience preferred.
• Experience with EHR systems preferred.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work independently and manage a patient caseload.
• Valid driver’s license and reliable transportation.

Responsibilities

• Develop, implement, and evaluate individualized care plans.
• Provide skilled nursing care in home setting.
• Educate patients and caregivers on disease management, medications, and care plans.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Supervise LPNs and support staff as needed.
• Complete accurate and timely electronic documentation.
• Ensure compliance with agency policies, Medicare regulations, and clinical standards.
• Participate in care conferences, team meetings, and quality improvement initiatives (QAPI).
