Job summary

St. Luke’s University Health Network in Bethlehem, PA seeks a Home Health Nurse to collaborate with an interdisciplinary team to deliver high-quality nursing services in patients' homes according to physician orders and organizational policies.

You will perform initial assessments, develop and adjust care plans, coordinate with physicians and caregivers, document thoroughly, and promote patient and family education while adhering to safety standards.

Qualifications

• RN licensed in the state of practice.
• Two years as a professional nurse preferred.
• Bachelor's degree preferred (BSN) if applicable.

Responsibilities

• Evaluate and perform initial and ongoing comprehensive home health assessments.
• Develop and adjust care plans based on physician orders and patient needs.
• Coordinate with physicians, referral sources, and multidisciplinary team.
• Teach, counsel, and promote preventive and rehabilitative measures for patients and families.
• Maintain and document patient records per agency guidelines.

Education

Registered Nurse license

Two years of professional nursing experience

BSN preferred
