Job Summary
This position collaborates with interdisciplinary team members to deliver high quality home health nursing services in accordance with physician orders and organizational policies. Services are provided in the patient’s home or place of residence.
Job Duties and Responsibilities
- Evaluates and performs initial and ongoing comprehensive assessments on patients referred for home health nursing services. Determines eligibility for admission, obtains financial and statistical data, formulates an initial care plan, and processes admissions.
- Refers patients to other agency disciplines and community resources as needed.
- Assesses, promotes, and maintains the health of individuals, families, and the community through teaching, counseling, and preventive and rehabilitative measures.
- Provides ongoing assessment to determine service frequency, changes to the plan of care, and discharge in compliance with agency policy.
- Functions as the designated coordinator of patient care, ensuring the provision of optimal, multidisciplinary care.
- Ensures care plans are followed according to agency standards, appropriate service utilization, and third‑party payor reimbursement requirements.
- Communicates with the patient’s physician, referral source, and multidisciplinary team orally and in writing as required.
- Maintains and uses patient clinical records, reports, and other written communications per agency policy.
- Records clear goals and plans with emphasis on family‑oriented care.
- Records reflect contact with physicians and agencies as needed; documentation is completed per agency guidelines.
- Participates in the development and operation of the agency:
- Keeping administration informed on changing community needs and lack of resources.
- Participating in the orientation and guidance of new staff.
- Interpreting agency services to families and community groups.
- Contributing to evaluation of service programs.
- Evaluating personal service effectiveness and contributing to staff development.
- Demonstrates efficient time utilization in the home, office, and during travel.
- Performs skills independently according to agency procedures and obtains needed information before visits.
- Develops and implements clear teaching plans, utilizing printed materials tailored to the patient’s diagnosis, needs, and abilities.
- Assumes personal responsibility to stay informed of current nursing care trends and professional standards.
- Participates in patient‑centered unit meetings.
- Maintains confidentiality of all materials and properly releases information.
- Complies with network and departmental policies on employee, patient, and environmental safety and reporting requirements.
- Demonstrates/ models the Network’s Service Excellence Standards (PCRAFT) in all customer interactions.
- Demonstrates performance improvement in clinical care/outcomes, customer service, operational systems, and safety.
- Demonstrates financial responsibility and accountability in daily procedures and practices.
- Complies with network policies regarding attendance and dress code.
- Demonstrates competency in assessment, treatment range, growth and development knowledge, and age‑appropriate communication.
Physical and Sensory Requirements
Physical Demands: Heavy Work (Exertional)- Occasionally lift or carry 100 lbs.
- Frequently lift or carry 50 lbs.
- Stand and/or walk at least 6 hours in an 8‑hour day.
Physical Demands: Non‑Exertional- Stooping (bending at the waist) and crouching frequently.
- Handling (grasping) frequently.
- Fingering (fine manipulation) occasionally.
Required functional capacity includes medium, light, and sedentary work. Good physical and mental health, neat appearance, and protection against body odors are expected. Adequate vision and hearing are required for comprehensive nursing care.
Qualifications
Education: Graduate of an approved nursing school and licensed as a Registered Nurse in the state of practice; Baccalaureate degree preferred.
Training and Experience: Two years as a professional nurse, or prior community health experience as a professional nurse, preferred.
