St. Luke's University Health Network seeks an experienced home health nurse to work with an interdisciplinary team in delivering high-quality nursing services in patients' homes and residences.

The role emphasizes comprehensive assessments, care planning, and collaboration with physicians, referral sources, and community resources, while maintaining thorough documentation and adherence to policies.

Qualifications

• Comprehensive knowledge of professional nursing theory and ability to apply it to the community health setting and Hospice.
• Ability to instruct professionals in the care and treatment of the physically and emotionally ill.
• Ability to prepare neat, timely, accurate and concise records and reports.
• Ability to accept and to utilize supervision.
• Knowledge of the use of community resources.
• Interest in keeping abreast of trends and developments in nursing practice.
• Possession of a current professional nurse licensure to practice in PA.
• Current CPR certification.
• Part time and per diem staff must possess a cell phone with text messaging.
• Position requires the use of your own private vehicle for Network business. You must have a valid driver’s license issued by the applicable state of residency, as well as evidence of auto liability insurance; which must have at least the state-minimum coverage limits.

Responsibilities

• Evaluates and performs initial and ongoing comprehensive assessments on patients referred for home health nursing services.
• Determines eligibility for admission, obtains financial and statistical data, formulates initial care plan.
• Refers patients to other agency disciplines and to community resources as needed.
• Assesses, promotes, and maintains health of individuals, families, and the community through teaching and counseling.
• Provides ongoing assessment to determine service frequency and discharge from service.
• Functions as the designated coordinator of patient care for multidisciplinary teams.
• Ensures care plans follow agency standards and third-party reimbursement requirements.
• Communicates with physicians, referral sources, and team members as required.
• Maintains patient records, reports and other written communications per policy.
• Records emphasize family-oriented approach to patient care.
