Job summary

Halcyon Home LLC seeks a dedicated Home Health Field Physical Therapist PRN to support geriatric patients in the Mesquite/Northern Kaufman, TX area. The role emphasizes compassionate care, autonomy, and teamwork with a strong, patient-centered approach.

The candidate will perform home health evaluations, develop plans of care, and coordinate with physicians, families, and caregivers to ensure safe and effective therapy in patients’ homes.

Qualifications

• One year or more of home health experience preferred.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage time effectively.
• Comfort with geriatric patient population.

Responsibilities

• Perform ongoing clinical assessments including OASIS when required.
• Develop and evaluate the plan of care with patient and caregivers.
• Document and complete clinical notes and orders in a timely manner.
• Provide supervision to assistants where applicable and coordinate with the care team.
• Instruct patients and families on device use and safety measures.
