Qualifications

Current PT license in Texas.
Experience in home health or related healthcare setting preferred.
Knowledge of Homecare Homebase EMR is a plus.
Strong clinical and assessment skills.
Excellent communication and interpersonal abilities.
Ability to work independently and manage time effectively.

Responsibilities

Performs ongoing clinical assessment, including relevant home health evaluations.
Develops and evaluates the plan of care in partnership with patient and caregivers.
Documents and completes clinical notes and orders in a timely manner.
Oversees appropriate patient care and coordinates with physicians.
Applies infection control and standard precautions during patient care.
Provides supervision to relevant therapy staff when applicable.
Plans and provides treatment for neuromuscular, neurological, and orthopedic disorders.
Communicates with physicians and other health care practitioners involved.
Meets continuing education requirements of the Agency and licensing board.
