Job summary

Mercy Health Home Care and Hospice by Compassus is seeking a Home Health Physical Therapist to deliver coordinated care across age groups. You will develop, implement, and evaluate plans of care in collaboration with the physician and the IDT to restore or maintain patient well-being.

The role requires an active Physical Therapist license, a BS in Physical Therapy, and strong communication with patients and families.

Qualifications

• Bachelor of Science in Physical Therapy required.
• Two years of experience as a Physical Therapist in acute care or rehabilitation preferred.
• Active and unencumbered Physical Therapist license in state(s) of employment.

Responsibilities

• Plan, implement, and evaluate patient care plans to restore or maintain well-being.
• Perform initial and ongoing clinical assessments and OASIS at appropriate times.
• Collaborate with the interdisciplinary team (IDT) and physician to coordinate care.
• Provide patient/family education and transfer/instruction on equipment use safely.
• Maintain infection control and compliant documentation in medical records.
