Job summary

Canyon Home Care and Hospice in Canyon, Colorado seeks a dynamic Physical Therapist to join our Northern Colorado team. You will provide 1:1 care to patients in their homes, create individualized plans of care, and collaborate with the interdisciplinary team to meet patient needs.

Qualifications

• Licensed Physical Therapist in the practicing state (or equivalent) per CAPTE-based requirements.
• Ability to provide 1:1 home health therapy.
• Strong communication with patients and interdisciplinary team.

Responsibilities

• Assesses patients in their homes and develops plan of care.
• Coordinates with family and agency personnel.
• Supervises PTA/HHA as applicable.
• Completes assessments and documentation per policy.
• Delivers skilled PT procedures and adjusts treatment as needed.
