Job summary

Qualifications

Responsibilities

• Perform services planned, delegated and supervised by the Physical Therapist.
• Prepare and maintain medical records and progress notes per policy.
• Instruct patients and families in home programs and activities of daily living.
• Coordinate therapy with the PT and IDT members, attend staff meetings.
• Maintain confidentiality and safeguard patient information.
