Job summary

Aspire Talent Solutions invites a dedicated Physical Therapist to join our home health team in the Denver area. This full-time position focuses on delivering compassionate, patient-centered care in patients' homes and documenting care per physician orders and policy.

The role requires a CAPTE-approved Master’s in Physical Therapy, an active PT license in our state, CPR certification, and a valid driver’s license to travel between locations. Community/home health experience is preferred.

Qualifications

• Master’s degree in physical therapy approved by CAPTE accredited program.
• Licensed to practice as a physical therapist in the state of operation.
• CPR certification to be maintained while employed.
• Must have a valid driver’s license and be able to travel to all business locations.

Responsibilities

• Provides patient care and documents treatment per physician orders, policy and patient needs.
• Performs patient evaluations and selects diagnostic materials to assess care.
• Informs physician of assessment results and recommended interventions before starting care.
• Develops and documents treatment plans, selecting interventions, materials and priorities.
• Provides organized, efficient treatment, including advising, educating and counseling patients.
• Presents patient information to physicians and keeps team informed of status and plan.
• Delivers treatment within agency and payer guidelines and patient needs.
• Sets objective, measurable goals in collaboration with team and patients.
• Meets agency productivity standards and uses work hours efficiently.
