Job summary

Enhabit Home Health & Hospice is seeking a Licensed Physical Therapist to provide skilled care to patients in home health settings. You will develop and implement therapy plans, evaluate progress, and collaborate with families and care teams to improve mobility and independence.

The role requires active state licensure, reliable transportation, and the ability to work across varying patient locations under physician orders and care plans.

Qualifications

• Must possess a valid state driver's license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Provide skilled physical therapy to patients to improve mobility and independence.
• Utilize physical modalities and exercises to support daily living and self-care.
• Implement specialty programs such as Balance and Fall Prevention and Joint Replacement programs.
• Educate families and health care team per physician orders and plan of care.
• Work under supervision of the Branch Director and follow established plans of care.
