This full-time role focuses on delivering patient-centered therapy in the comfort of patients' homes. You will assess, design and implement personalized rehabilitation plans, educate families, document progress, and collaborate with a multidisciplinary team to support safe, effective recovery.

Qualifications

• Master's degree in physical therapy from a CAPTE-accredited program.
• Current Colorado PT licensure and CPR certification.
• Willingness to travel to patient homes throughout Colorado.

Responsibilities

• Perform comprehensive physical assessments and analyze medical histories to evaluate patients' conditions accurately.
• Create and implement tailored treatment plans that address rehabilitation goals.
• Conduct therapeutic exercises and training programs to improve mobility and functionality.
• Educate patients and families about injury prevention, wellness strategies, and the recovery journey.
• Monitor and document patients' progress, adjusting plans as needed to meet goals.
• Collaborate with a multidisciplinary healthcare team to ensure holistic patient care.
• Maintain meticulous patient records and documentation in compliance with standards.
