Job summary

AccentCare seeks a dedicated Physical Therapist to deliver evaluation and treatment to patients in home health across Lehigh/Northampton counties. You will collaborate with an interdisciplinary team and document care according to policies and standards.

The role requires a Master’s in Physical Therapy, PA licensure, CPR certification, and willingness to travel to patient locations. You will develop plans, educate patients and families, and adjust interventions to meet goals.

Qualifications

• Master's degree in physical therapy approved by CAPTE accredited program.
• Minimum nine months experience as a physical therapist.
• Licensed to practice as a physical therapist in the state of operation.
• CPR certification maintained during employment.
• Must be a licensed driver able to travel to all business locations.

Responsibilities

• Provides patient care and documents treatment per physician orders and policy.
• Performs patient evaluations and selects appropriate diagnostic materials.
• Develops and documents treatment plans and priority goals.
• Maintains organized patient documentation and communicates status to team.
• Travels to patient locations and adheres to productivity standards.
