Job summary

AccentCare is seeking a Physical Therapist for Home Health in Texas. The role involves evaluating and treating patients in home health, coordinating with an interdisciplinary team, and educating patients and families.

The PT will manage a caseload, document care, and participate in conferences and in-services. Requires CAPTE-accredited PT program, Texas license, CPR, and ability to travel to patient locations.

Qualifications

• Master’s degree in physical therapy (CAPTE-accredited program) required.
• Minimum nine months experience as a physical therapist.
• Licensed to practice in the state of operation (Texas).
• Must possess and maintain CPR certification.
• Must be a licensed driver who can travel to all locations.

Responsibilities

• Provides patient care and documents treatment per physician orders, policy and patient needs.
• Performs patient evaluations and selects appropriate diagnostic materials.
• Informs physician of assessment results and recommended interventions.
• Develops and documents treatment plans and priorities.
• Delivers organized treatment, educating and counseling the patient.
• Presents patient information to physicians and keeps the team informed of status and plan.
• Delivers care per patient needs and payer guidelines.
• Sets objective, measurable goals with team and patient.
• Meets productivity standards and uses time efficiently.

Education
